Pharmacist on the ward
The pharmacist on the ward sees himself as a member of an interprofessional team on the ward and assumes joint responsibility for drug therapy and drug therapy safety in the hospital.
Pharmacists on the ward are responsible for providing individual drug-related advice to the doctors and nursing staff working there. They look after hospital patients with regard to their drug therapy during their stay in the hospital - from admission to discharge. The pharmacists optimise and evaluate the individual drug therapy with regard to its appropriateness, effectiveness, safety, efficiency and patient adherence.
Pharmacists on the ward support the work of doctors and nursing staff with the following pharmaceutical services:
As part of patient admission:
- Pharmaceutical drug anamnesis with included drug therapy safety (AMTS) check
- Changeover of home medication to preparations on the hospital's internal house list with corresponding electronic documentation
- Pharmaceutical counselling of the patient
As part of the inpatient stay:
- Pharmaceutical ward rounds with AMTS testing, therapy recommendations in accordance with the currently valid therapy standards
- Counselling, training and support for all professional groups involved in the prescription, procurement, proper handling and safe use of medicines
- Creation and implementation of in-house guidelines and standards for drug therapy
- Monthly drug consumption analyses
As part of the patient's discharge:
- Preparation of discharge medication for the doctor's letter with changeover to home medication and AMTS review
- Provision of the current medication plan
